As a new patient, please plan to be in our office 15 minutes before your appointment, in order to obtain all necessary forms and information, including your medical history. Plan to be in our office 60-90 minutes for your first appointment.
For your convenience, some of the forms needed are available here for completion in the comfort of your own home. Please print them and bring them with you, as well as your health insurance card, your photo ID, and a list of your current medications.
